| Making appointments Patients are seen by appointment only. Our appointment desk is open Monday through Friday from 8 a.m. until 5 p.m. For your convenience, evening appointments are available. The initial visit On your initial visit, we will begin a medical record file. It is highly recommended that you bring any X-rays, MRIs, Cat Scans or EMGs that have been performed in relation to your injury or condition. We participate in a number of health plans, so please be certain to bring your health insurance card on the initial visit. Otherwise, you will be given receipts that can be submitted to your insurance company for reimbursement. Prescriptions and renewals All prescriptions and authorizations for renewals should be requested between 8 a.m. and 5 p.m. For your protection, evening and weekend renewals will not be accepted because the doctor must have your chart to renew your medication. Surgery If surgery is indicated, your operation will be scheduled at one of the following hospitals: North Ridge Medical Center, Holy Cross Hospital or Imperial Point Medical Center. The facility for your surgery will be determined based on your insurance coverage and the type of surgery to be performed. For the locations of all three facilities, please see our map. Referrals If you have been referred to us by your Primary Care Physician, please let us know so we can coordinate your care. X-rays If you want copies of your X-rays, please inform the technician prior to the X-rays, and they can be provided for a small charge. Original records cannot be released.
